Children Leaving Care team – Bradford

Taking care of the health needs of Bradford children and young people aged 16-25 who have been looked after or are homeless/unaccompanied asylum seekers.

Our Leaving Care nurses work closely with social care to look after the health needs of Bradford children and young people aged 16-25 who have been looked after or have presented as homeless/unaccompanied asylum seekers.

We have a named nurse who overseas the whole service, Andrea Hayley, who ensures children and young people are receiving the highest quality care available to them.

Our team of nurses can:

  • offer you a health assessment carried out by a doctor when you first become looked after;
  • design a health care plan for you, following assessment, which can be shared with other health and social care professionals and used as part of your ongoing care plan;
  • keep an updated health history for you based on the health assessment;
  • give you advice and time to talk to a nurse about any health issues that are worrying you at any time;
  • offer continuity of care by aiming to provide a nurse who will remain your single, consistent contact for the whole time you are being supported by the Leaving Care service;
  • refer you to specialist services if necessary;
  • provide you with public health advice and information.

How can you access the Leaving Care team?

A health professional or a member of Bradford Council Children’s services can make a referral to see our nurses on your behalf.
