Your health history is important. Health professionals may need to view a record of your previous care and treatment so they can provide safe and effective care and treatment for you now and in future.
In the past, young people aged 16 and over were given a paper ‘heath passport’ with a record of their health history. We know these often got lost or damaged. That’s why now, if you are 16 and over, you might have been invited by our Children in Care service to register for an online health passport.
Having an online health passport means you can easily
It also reduces the risk of your health passport being lost or damaged and supports our Trust in using less paper.
The NHS has been working with Patients Know Best (PKB), a social enterprise and technology platform. They work to give people safe, secure access to their health information. To register for an online health passport you need to create an online account with PKB. This is free, quick and easy to do.
Soon the NHS app will be linked with your health passport held on PKB. You can find out more about that here. For now, all you need is a PKB account.
Once you have registered you can log in to your online health passport anywhere, using a smartphone, tablet or computer with an internet connection.
Your child in care nurse will also be able to look at your health passport online. This helps us to work together to improve your care, using your up-to-date health information.
